Medications Used for Drug and Alcohol Addiction Detox Programs

When some individuals think about medication-assisted treatment options, their minds jump to methadone and buprenorphine. The truth is that these are just two medications that can be used to support those who are experiencing withdrawal. Determining which medication is the best fit is dependent on personal characteristics including the severity of addiction, associated symptoms, and other present risk concerns.

Opioid Detox Medication

Several medication options can be used to support those who are living with an opioid use disorder. One of the most commonly used medications is methadone, which is a medication typically dispensed by a provider daily. Individuals who progress in their recovery may be eligible to receive two doses at once with the expectation that the second would be used the following day as prescribed. Individuals who choose this medication route often participate in routine urinalysis screenings to ensure compliance with their medication regimen. This medication can be effective for those who are living with a severe opioid use disorder. 

A second option is buprenorphine, which is available in sublingual films, transdermal patches, and injectable ampoules. This medication is a partial opioid agonist, which is typically taken via sublingual film under the tongue. This medication may be a better fit for individuals who have mild to moderate opioid use disorder. Naltrexone is another medication option; however, it is not typically used on its own. As an opioid receptor antagonist, it blocks your ability to experience euphoria from opioids including heroin, Suboxone, and methadone. Naltrexone has not had great retention rates, which is often attributed to the side effects and risks of the medication such as dysphoria, depression, and insomnia. Naltrexone can be effective in reducing the severity of opioid cravings while in recovery.

Alcohol Detox Medication

The use of MAT with alcohol detox programs is common, but not always necessary. The use of MAT is determined on a case-by-case basis and is known to not be a good fit for each individual. Benzodiazepines are a well-established MAT option for those who are experiencing withdrawal symptoms from an alcohol use disorder. These medications can be used to reduce de novo seizures that may occur during withdrawal, and are typically used on a short-term basis. Anticonvulsant’s are another medication option that can be used to mitigate the risk of seizure, improve sleep, decrease cravings, and reduce psychological distress, without the respiratory risks associated with benzodiazepines. 

Another MAT option, acamprosate, works by reducing the amount of glutamate present in the brain, reducing withdrawal symptoms, decreasing cravings, and enhancing treatment outcomes. Some research has supported that this medication should be started before or at the very beginning stages of an individual detox for optimal support. Disulfiram, commonly known as Antabuse, is another commonly used MAT for alcohol use disorder. This medication works a bit differently than others because it does not have any impact on an individual’s experienced withdrawal symptoms. Rather, this medication is used to deter future use of alcohol by causing unpleasant substances if a person were to drink. The caveat to this medication is that you have to take it consistently, as prescribed, to develop the needed build-up of acetaldehyde within your body. If an individual misses or skips a dose, the functionality of the medication will be lacking. 

Benzodiazepine Detox Medication

The detoxification process for benzodiazepines looks a bit different from other substances because the medications used to treat this addiction are often longer-acting benzodiazepines. This is not a long-term treatment option because there would be a significant risk of misuse and abuse among those living with an existing benzodiazepine addiction. Rather, the long-acting benzodiazepines will be tapered down while being actively monitored by healthcare professionals. Psychotropic medications may be available for those who were using benzodiazepines to cope with mental health distress, including anxiety and panic attacks. 

Stimulant Detox (e.g., Cocaine, Methamphetamine) Detox Medication

Without any FDA-approved medications for stimulant detoxification, detox care programs utilize a comprehensive approach by using medications to treat specific symptoms that individuals are experiencing, such as vomiting, nausea, muscle discomfort, and insomnia.
